Efficacy of Cefotiam Hexetil in Bacterial penumonia



Abstract
Cefotiam hexetil is an oral cephalosporin and is a prodrug of cefotiam which has been synthesized by esterifying cefotiam at the carboxyl group. The present study was designed to evaluate the efficacy, safety and bacteriological response
including
MIC.
Study subjects included the patients who were diagnosed as acute bacterial pneumonia. There were 19 acute lobar and bronchopneumonia without underlying diseases and 12 acute pneumonia with underlying diseases. The ratio of male to female was
14:17
and
the average age was 54 14.8. Underlying diseases were inactive pulmonary tuberculosis (5), endobrochial tuberculosis (1), COPD (4), bronchial asthma (1), bronchiectasis (3).
@ES The results are summarized as follows:
@EN 1) Clinical improvement was present in 78.0% of fever, in 52.6% of cough, in 100% of sputum amount, in 93.7% of sputum character, in 78.9% of dyspnea, in 100% of chest pain and in 93% of rales.
2) On laboratory findings leukocytosis was improved to normal range in 54.8%. There were no changes in hemoglobin, hematocrit, the number of platelet, GOT, GPT, Alkaline phosphatase, total bilirubin, serum creatinine, BUN, urinary protein and
sugar.
3) On bacteriological identification and culture, pathological bacteria were found and the MIC of cefotiam for 59 patients showed that ¥â-hemolytic Streptococcus, Klebsiella pneumonia, Acinobacter calc aceticus var anitratus, Escherichia coli are
sensitive but Pseudomonas aeruginosa, Serratia liquefaciens, Xanthomonas maltophilia, Staphylococcus aureus were resistant to cefotiam.
4) The most common side effect was dyspepsia and nausea which were reported in 6.45%.
5) Cefotiam hexetil treatment was judged as fairly useful in 67.7%, slightly useful in 19.4%, not useful in 12.9% of the patients with bacterial pneumonia.
